Go to www.stilllifenyc.com, they have a bunch of standard ones (all types), or you can contact them and they'll make one custom for you for a few extra bucks. Not cheap, but best quality hat I've seen for sure.

doctorgnar, I got both shoes from eBay. I missed out on them once and when they came around again I had to get them. I don't know what size you are, but there's a pair in 8US for $35 BIN. I got mine in an 11.5 and the seller had a 12, which I'm not sure he sold. Send me a pm if you want the ID, though it may be a long shot.
