ok, i will heading to the US soon, so how much should one tip, say for a normal lunch/dinner at a normal cafe/restaraunt?

down here in Australia, i'd only tip for excellent service and food, which is rare

If you tip 15% consistently and across the board, no server will ever hate you. No one will ever love you either but hey, that's life.

1. tip the shampoo woman at the salon?

2. tip furniture delivery people?

3. tip repairmen?

4. appropriate tip for happy ending?

5. in countries such as japan there is no tipping. is that because they will be polite to you even if there is no incentive to be?
