At noon on Wednesday, after 26 hours of surgery, a team of doctors in New York finally managed to remove Robert Collison’s liver. The only thing harder would be putting it back into him.

A huge cancerous tumor in the back of Mr. Collison’s abdominal cavity had attached itself to his liver and engulfed other organs and blood vessels. Chemotherapy and radiation had not fazed it. He lives near Milwaukee, and doctors there said the tumor was inoperable and would kill him. But Mr. Collison, 59, with five children and nine grandchildren, was not ready to give up.

“I can’t leave yet,” he said in an interview the night before his operation. “I’ve got too much fun ahead of me.”

He had come to NewYork-Presbyterian Hospital/Columbia University Medical Center to be treated by Dr. Tomoaki Kato, one of the few surgeons willing to take on such difficult, risky cases. Dr. Kato is an expert in ex vivo resection — taking out organs and operating on them outside the body to remove tumors that cannot be treated in any other way, and then sewing the organs back in. The procedures are grueling surgical marathons that can easily last 30 hours or more, involve a dozen surgeons and anesthesiologists and cost more than $300,000.
