Incu has an A.P.C. black blazer (part of a suit) in small at the Galeries Victoria store, you can check it on the website. I've tried it on and the length fits quite short on me and I am only slightly taller than yourself. However, it costs $699 but it fits the bill for what you are looking for in a black blazer.

First off, it's the hangtags, which said it was actually a longsleeved black tee, also it also had the superfluous hangtag (the one which says "the garment is made from a delicate fabric/knit and handle with care", which is never on the dress shirts, only on the "distressed" knits, jeweled/beaded/embroidered items or some jeans).

Also, Dior's never done a dress shirt with such a wide collar (looks like almost a spread collar).

the blazer + jeans combo is a hard look to pull off. I don't think I've ever seen it done well.
